2.            How to Nominate – Parents, adult family members or teachers who know a student whom they feel demonstrates an aptitude toward science and learning must log on to www.kljb.com, the Fox 18 Facebook page, or www.kljb.biz to access the nomination page.  Fox 18 will require the adult nominator’s name, city, zip, phone number and e-mail address, plus the nominee’s name, grade, school, and a brief explanation of why they should be chosen the “COSMOS Student of the Week.”  NO information regarding the minors being nominated will be posted on our web site.  All nomination information will go directly to Fox 18, which will keep all information secure.

3.            Promotion Dates – Students can be nominated any time from Thursday, February 27th through Wednesday, June 4th, 2014.  End date is subject to change. 

4.            Selection of Promotion Winners – Each Wednesday at 12pm noon Central Time, Fox 18 will select a nominated student from all entries received at that time.  A reporter from the Fox 18 Nine O’clock News will shoot a brief story about the student and his/her teacher & parents on the following Friday, then air the story in the Fox 18 Nine O’clock News that Sunday night following “COSMOS: A Spacetime Odyssey.”

5.            Prizes – Thirteen (13) students will each receive a “COSMOS Student of the Week” plaque, a one-year Family Membership pass to the Putnam Museum, and an Under Armour gym bag from Prairie Farms Dairy.  Total value of each prize - $180.

6.            General Conditions – No purchase necessary.  Purchase will not improve an entrant’s chances of being selected.  Fox 18 will not be responsible for loss of service to web sites due to power loss or any other reason that may prevent any individual from submitting a nomination.  All viewers who have won ANY Fox 18 or The Quad Cities’ CW contests are ineligible for 90 days from the date of that contest’s completion, NOT the date of their winning. By entering each entrant accepts and agrees to be bound by these rules.  Decisions of Fox 18 shall be final and binding.  Taxes are the sole responsibility of the nominees.  Prizes cannot be transferred, redeemed for cash or exchanged.  No substitute prizes, except at the discretion of Fox 18, will be allowed.  By entering, each contestant consents to the use of his/her voice, name and likeness for broadcast transmission, advertising and promotion in connection with the promotion.  Fox 18 makes no warranty, representation or agreement that any videotape or likeness of the winners will be broadcast or otherwise used in any manner.  The winners must execute an affidavit of eligibility, liability release and/or publicity release and any other documentation that Fox 18 requires.  Failure to claim prize within ten (10) business days of fist notification of winning from a Fox 18 representative shall result in forfeiture of the prize and an alternate winner will be selected.  If an alternate winner cannot be chosen before the promotion ends, the prizes will not be awarded.  Void where prohibited by law.  All federal, state and local laws apply.  Fox 18, The Quad Cities’ CW, the Putnam Museum, Western Illinois University, Prairie Farms Dairy, their parent, subsidiary, affiliated and related entities, and the officers, directors, employees, licensees and assignees of each accept no responsibility or liability in connection with any injuries, losses or damages of any kind caused by or resulting from the acceptance, possession or usage of any prize awarded hereunder.  Fox 18 reserves the right to revise and update the rules without advance notice.
