1.            Prizes – One winner will receive a 4-pack of V.I.P. vouchers to the Disney on Ice show, coming to the iWireless Center in Moline, IL November 14th through November 17th, 2013.

2.            Eligibility – To be eligible to participate, viewers must be eighteen (18) years of age or older and reside in the KLJB-TV viewing area, which consists of the following Illinois counties:  Bureau, Carroll, Hancock, Henderson, Henry, Jo Daviess, Knox, LaSalle, Lee, McDonough, Mercer, Ogle, Rock Island, Stark, Warren and Whiteside; and the following Iowa counties: Cedar, Clinton, Des Moines, Henry, Jackson, Lee, Louisa, Muscatine and Scott.  Employees of KLJB-TV, KGCW-TV, their parent companies, advertising agencies, fulfillment companies, printers, distributors and mailers and the immediate families (or non-family members of the same household) of each are not eligible to play.  The term “immediate families” includes spouses, grandparents, parents, siblings, children and grandchildren.  The term “non-family members” include boyfriends, girlfriends, roommates or tenants at the same address. 

3.            How to Play – No purchase is necessary to play.  Purchase will not improve an entrant’s chances of winning.  To enter, log onto www.kljb.com and upload an image of your child wearing a Halloween costume.  You will also need to provide us with your contact information (name, address, city, state, zip and phone number).  Or mail your contact info to “Disney on Ice Photo Contest” care of KLJB-TV 937 East 53rd Street, Davenport, IA 52807.  Entries can also be hand-delivered to KLJB-TV using the same criteria as mailed entries.  Only one entry per person is allowed.  Multiple entries from the same person will be disqualified and only the initial submission will be allowed.

4.            Contest Dates – Entries will be accepted beginning Monday, October 21st and the Station must receive all entries no later than 11:59:59pm on Sunday, November 10th, 2013.  The winner will be selected and notified the following Monday, November 11th, 2013.  It is up to the winner to claim and redeem their vouchers as quickly as possible to ensure that they are able to attend the November 14th show.

5.            General Conditions – No purchase necessary.  Purchase will not improve an entrant’s chances of winning.  KLJB-TV will not be responsible for loss of service to internet due to power loss, service interruption or any other reason that may prevent any individual from completing his/her entry.  All viewers who have won ANY KLJB-TV or KGCW-TV contests are ineligible for 90 days from the date of that contest’s completion, NOT the date of their winning.  If it is determined that a winner falls into this 90 day lock-out period, they will be disqualified and another winner will be chosen.  ***EFFECTIVE JANUARY 1ST, 2011, ALL GRAND PRIZES WINNERS ARE INELIGIBLE FROM WINNING ANY CONTESTS WITH KLJB-TV OR KGCW-TV FOR ONE HUNDRED AND EIGHTY DAYS FROM THE DATE OF THEIR WINNING**  KGCW-TV will not share any collected Personally Identifiable Information (PII) with outside 3rd party sources without an entrant’s written consent.  By entering, each entrant accepts and agrees to be bound by these rules.  Decisions of KLJB-TV Fox 18 shall be final and binding.  Taxes are the sole responsibility of the winner.  Prizes cannot be redeemed for cash or exchanged.  By entering, each contestant consents to the use of his/her voice, name and likeness for broadcast transmission, advertising and promotion in connection with the contest.  KLJB-TV makes no warranty, representation or agreement that any videotape or likeness of the winners will be broadcast or otherwise used in any manner.  The winners must execute an affidavit of eligibility, liability release and/or publicity release and any other documentation that KLJB-TV requires.  Void where prohibited by law.  All federal, state and local laws apply.  KLJB-TV, KGCW-TV, their parent, subsidiary, affiliated and related entities, and the officers, directors, employees, licensees and assignees of each accept no responsibility or liability in connection with any injuries, losses or damages of any kind caused by or resulting from the acceptance, possession or usage of any prize awarded hereunder.  KLJB-TV reserves the right to revise and update the rules without advance notice.
